eAYSO Registration Process
Important
points to note before beginning the process:
Pre-registering
your child will be a two step process:
Step 1 – You (the parent) will have to register
yourself within the “eayso” site to allow you access to the site through a
“user name” and “password”. Once you have these two items, you
can do Step 2.
Step 2
– Register your child separately to complete
a separate registration process as a volunteer.
The
on-line process is relatively straightforward and does not take much time.
Once you have completed the process, registrations in future years will be
extremely easy since all information previously entered will already be in the
system.
IMPORTANT
NOTE: Even if
you pre-register on-line, you will still need to come to AYSO registration to
give us your three signed copies of the on-line player registration form
(along with your payment) and/or to give us your completed volunteer
registration form. However, by doing the pre- registration on-line, you
will be in and out of registration in no time, and future registrations will
also be much faster than before.

bring up a separate window allowing you to switch back and forth between these
instructions and the actual registration site.
Step
1 – Initial Registration for Site Access
(required prior to registering your
child)

to Sign Up for a Username & Password
The
next screen is the Member
Registration form. Provide your
name, contact information, and your e-mail address. Choose a username
and a password.
After
entering the zip code, it tells you to press Tab and that the City and State
will appear in the appropriate box.
On this form,
and for all others on this site, don’t tab after each portion of a
phone number. Just enter the area code, and the cursor will
automatically jump to the next field. Then, enter your three digit phone
prefix, and it will automatically jump again.
Once you have
completed the Member Registration, a screen will appear which states:
“Thanks for Registering.” Please note that, at this point, you have
not completed the registration process. All you have done is obtain a
user name and password to allow you to access the site. At the bottom of
this page, there will be a “back” link.
Click
on the word “Back”. It will
take to back to the original sign-in page. Now, log into the system with
the username and password that you just set up in Step 1.
You
will then be taken to the Welcome screen which will display your name.
Click on the “My eAYSO“ link in
the upper left portion of the screen.
Click
on: View/Edit
Parent Information
This will
bring up a page with the registration information which you just entered.
Please review the information to ensure that it is correct.
Important:
You will need to fill in the required field labeled “Relationship
to Child”; Father, Mother or Guardian. It is marked with a red
asterisk and must be filled in before the system will allow you to register a
child.
You will have
the option of entering information for a second parent. All parent
information on this page (either for one or two parents) is automatically put
into the child’s registration form.
Click
“Save” at the bottom of the page.
This will again take you back to the Pre-Registration menu. You can now
pre-register your child. If you are registering more than one child,
each one must be registered separately.
Step 2 –
Registering your Child
To
pre-register your child, click on “Add a Player”
on the pre-registration menu.
Select
the Region. (Mililani is Region 119)
Answer
“yes” or “no” to the question whether your child has played with
AYSO before. If you wish, enter the additional information
requested. (As long as the field does not have a red asterisk next
to it, it is not required to be filled out.)
Click
Next.
Enter
your child’s details. Remember to complete all fields marked with
a red asterisk.
When
you enter your child’s birth date, the program will automatically
calculate and insert your child’s “AYSO age."
After
completing all fields, click “Submit” at
the bottom of the page. You will be given three choices – Add
one more player; Become a Volunteer
and Return to main menu.
If you
wish to register another child, click “Add one
more player”. If you wish to volunteer, click “Become
a Volunteer” and then see instructions below for registering as a
volunteer.
Step 3 – Registering as a Volunteer
To
pre-register as a volunteer, access the data entry page by either clicking “Become
a Volunteer” at the end of registering your child or by clicking
“Become a Volunteer” on the pre-registration menu on the MyeAYSO
page. Complete the required fields for the volunteer form. All
fields marked with a red star are required fields.
Once
you have entered the required information, click “Submit”
at the bottom of the page.
Step 4 – To Complete the Registration
Process
Once
you have entered the information required for either player or volunteer
registration, you will need to print your registration forms.
Go back
to the pre-registration menu on the MyeAYSO
page.
Click
on: Print Pre-Registration Form.
Select
Player or Volunteer form and click on print/preview forms. Please
check the contents of the form before printing. If you see anything
that needs to be changed go back to the main menu of the pre-registration
page and click on the View/Edit option. Correct the data,
then select the “Print Pre-Registration” option again.
Once you see the form, you will
see a print icon on the top left corner. If you have a printer attached
to your computer, click on the printer icon and print the form. You can
also save the form in your computer to print it later.
NOTE:
You need to have Acrobat Reader in your computer to print these forms.
If your computer does not have Acrobat Reader, click on the Acrobat link and
download the Reader. Forms can be downloaded and printed only after all
of the required information. Use the View/Edit option in My eAYSO to update player or volunteer
information and print the form again.
For
player registration, please print three copies of the registration form,
sign all three and bring them to registration along with your payment.
For
Volunteer registration, print two copies of the form, sign both and bring
them to registration along with your driver’s license.
Again,
it is important that you bring these forms to registration with your payment.
By doing this pre-registration in advance, you will go through the
registration process extremely quickly.
