The Final Winter and Spring 2011 Registration:
At Mililani Ike Elementary School:
Wednesday November 10, 6pm to 8pm
Cost: $65 for current Fall Season Players, $75 for new players
Age Groups: The U12 and above age groups will be registered with the Age Group Coordinators
Winter season (December 2010 to March 2011):
U8G, U10B/G
Spring season (March 2011 to June 2011):
U6B/G, U8B
Go to www.eayso.org to print out three (3) copies of your registration form.  Registration cost is $75.  If you bring less than three (3) copies you will need to fill out a handwritten form.

Age Range: For Children Born Between 8/1/1991 and 07/31/2006.  Fall season practices start August 2010 and games complete either the first or second week of November 2010.  The below is for the 2010 season:

For NEW players, please bring the following:
An original copy of you child's birth certificate or other proof of child's age.
Emergency contact information.
Doctor and medical insurance information.
Method of payment.
